As above its a crack.
With no clues as to process, material, consumable, conditions, how can anyone be able to work out what's gone wrong.
Cracks can come from many many possible reasons.

looks a bit like a -concrete floor that's had lino stuck down to it and pulled up 2 me .it was posted at 5.29 pm and he left at same time .why ask

Going by the scale and imaging details. X-Ray from scanning electron microscope and the whole image is just over 1mm wide

Hydrogen embrittlement, stress corrosion cracking. I dunno just like the sound of terminology

Inclusions cause cracks, crators cause cracks, solicitation (cold) cracking, hot cracks, cracks caused from improper pre or post weld heat treatment (stress cracking) some materials just are not weldable with high high amounts of carbon will crack with welding. Cold fast welding such as tack or even arc strikes cause cracks from fast cooling and become hard and brittle.

Unless I am very much mistaken, that's a continental transform fault; a tectonic boundary between two continental plates with a right-lateral strike-slip (horizontal) movement.

It not "wrong" as such, even if it is a "fault". Just don't build a house on it.
