BOC Valve - opinions please!

Ok folks, I'm new here and just wanted to get an opinion on the state of the valve on my BOC-supplied Argoshield cylinder shown below. With the regulator in place and tightened (not too much) there is a substantial leak at the joint to the cylinder. The regulator nut sits proud by about 7mm when tight and if you look into the neck of the cylinder there seems to be a small crack.

(1) Is, as I suspect, the crack most likely to blame?
(2) Does the regulator need any sort of sealing washer where it joins the cylinder?
(3) If the crack is to blame what are my chances of getting BOC to replace the cylinder

I've had the cylinder three weeks now, the first regulator leaked in the same way so I treated myself to the one shown (with the same leaky results). Only then did I inspect the valve to find the crack.

Any input much appreciated.

Valve nut sitting proud is normal. I normally give the spanner a slap to ensure it is tight, if it still leaks and the bullnose looks ok on your reg get boc to swap it: never had any issues with them swapping cylinders.
Valve does look a bit hammered

Tag it (stick a note on it) and back it. No sealing washers (or anything on the threads) should be used, the gas seal is made by the radiused seat.

As Paul never had any issues swapping out dodgy cylinders, may not get a straight answer from the BOC agent but typically will get a letter from BOC once they've got the cylinder back to let you know you won't be charged

I had a dodgy one the other week it didnt leak but when the valve was open the gas didnt come out , they changed it no problem , they will deliver new and take the old !
