College Assignment on Welding Mask - Survey

I am currently studying Mechanical Engineering and one of my lessons is to create a project that relates to Engineering in some way. I enjoy welding and decided to create a welding mask that is modifed in a different way.

I need to collect some data from welders and this forum seemed to be quite active so I thought I could ask some of you to complete this simple five question survey!

Done, personally I would have had question one as a list in order of preference rather than just choose one. I would be surprised if he is getting paid it wasn't a professionally crafted market research survey.
