mine has been crashed at some point (on a different lathe) and ok but not ok
Why not try cutting the 4 jaw with a carbide boring bar.

Yes, no probs but use a fairly big nose rad negative insert as it will stand up better to the interrupted cut.
If it is the chuck body that is knackered then it may not really help much as it won't be uniform over the capacity of the chuck but I suppose you have nothing to lose if it is knackered anyway. Just remember, as with soft jaws, you need to load the chuck jaws whilst boring them.

Depends if it's the jaws that are bell-mouthed or the chuck. My bet is on the chuck especially if as you say it seems to have done little work. Hardened block of steel jaws vs soft cast iron chuck body, it's not the jaws that are going to wear.
Remove the jaws and run a dial gauge along the guides in the slots on the chuck face. I'm guessing you'll find they are not straight and flat. If you have a surface plate you can sit the chuck down on that face-up and literally map the whole lot with a surface gauge with a 10th-reading DTI. Any deviation from flat will show up right away.

Even knowing where the issue is I'm not convinced it is fixable, as it's only an issue with small section that can pass through the bore rather than sit against the face of the chuck I can live with it
