Based on this photo I am wondering if there was some dirt or other debris trapped in the joint when it was nipped up.
That would have held the two parts apart (allowing corrosion on the face circled in red), whilst allowing the bolt to be correctly torqued (meaning the face circled in green remained clean).
When whatever debris was in there finally collapsed due to the stress, the bolt was suddenly no longer correctly torqued and rapidly worked loose.
