Very unlikely it's worn, if you're seeing a change in print parameters it's most likely due to a new reel of filament.
If you're printing exotic filaments hardened steel nozzles are available.

Not all nozzles are the same. It depends on the hot end you are using more than the printer unless it's truly standard.Any brass nozzle will fit, 0.4 seems the standard although wood/carbon fibre/glitter etc may need a larger nozzle to prevent clogs.
Smaller nozzles will give finer details but extend print time hugely depending on how small you go.
I have a range of nozzles from 0.1-0.8 and only ever seem to use 0.4...

Photo taken part way through the print. For a start the infill looks a bit straggly here and there. Also the screw holes weren't forming properly. I mean look at this top right hole, it's not like that on the model:

Tried slowing the print speed to 30mm/s from 50mm/s. Top hole still missing material and straggly:
Still too fast a print? This is on for about 6 1/2 hours.
Any ideas?
