I appreciate RFA is different, that was just an example off the top of my head - but I’d beg to differ that a ship yard with little or no experience in building cranes would build and install one - mind you Liebherr have plenty of experience and it didn’t do them much good

Yeah i get that but most of these yards have plenty of experience building ultra large cranes for ships, i have seen them being built. They delivered from a Shanghai yard back in 2016, completely built in china a 5000 ton heavy lift ship so i am very perplexed as too why they didnt do it them selves.

Any way back on track.

Perhaps the water suction was miss calculated

Probably though the hook as had a casting fault which has gone undetected by ultra sound or xray, which has then caused failure when sufficent load was applied.

It beggars belief how a simple error can destroy millions of quid in a second.

The Victorians didn't understand health and safety. They built everything big and then massively over engineered it

I was blaming the lack of it in this case.
Ok so it was a test but a huge failure like that could have killed people down the line.

Yes you are possibly right.

Germans are the worst engineers and least efficent people i have ever come across bar the americans. When we call in germany we are always delayed by container gantry cranes breaking down and having too be repaired. When i was a cadet i signed off my ship in dubia it was on its way to europe when it got too bremerhaven a container crane colapsed on to it into the hold killing the operator and causing a huge fire. Thank goodness it was only the operator.

Looks like a few folk have got it wrong: when I used to design anything critical to life, it was checked by a colleague or my boss, then the design calculation package sent to the likes of Lloyds or DNV for sign off.

Should have too have gone too the ships insurance which was probably loyds or dnv gl so multiple people screwed up if it was a calculation error.

up till recently theres a building down Middlesbrough that did break stress testing on metals was well used by firms all over the country dunno if its still there but hasn't been used in donkeys years

I’ve studied H&S, worked in it all my life, yet I’ve never ever met a H&S person that could sign off the design for a crane

Absolutely, 73 were reported to have died building the Forth rail Bridge: there are memorials at both ends to them, though they are quite recent.
