She was to go on a job in Scotland, not oil related wind farm job!This may sound harsh, but in some oil industry office, folk will not be unhappy about that mishap. It looks like a heavy lift support ship, for lifting in big subsea modules offshore, but as prices are so low, deep water subsea stuff isnt good economics at the moment.
It will have been pre booked, so the dealys will mean some poor names at Lloyds insurance will end up paying out. And liquidated damages for delays paid others too.
And no matter how many layers of protection you put in place, failures still seem to happen, just the outcomes are not as severe as they were in victorian times.

Very very common occurrenceA preserved steam railway in Estonia paid a lot of money for German certified steam boiler tubes when they rebuilt a boiler, did not want Chinese, the tubes failed after 3 or 4 years, when tested they were found to be way under spec. and 99% likely Chinese but supplied with German paperwork from a German supplier...………………….. the case is ongoing.

You’ve got very very clever people, surrounded by muppets. Some of the best welders around, tied down by ridiculous rules and regulations.
I’m sure I’ve put this on here before but some time ago, they did a study at BAE.
The guys would get to work, sign out their welding consumables, by the time they got setup and welding, they done about 20m - half hour and it was tea time. Then they had to sign out fresh consumables again, then it was lunchtime, etc etc.
Everyday for two men, 3 hours of welding got done!
Produce or pronounce?they demanded tolerances you couldn’t pronounce !
I dont think the hook broke, looking at the pics it looks like the hooks swivel housing has failed.

Same lousy, stupid, egotistical management though! Still passing the buck downwards.
I would guess at some lousy planning and project management, especially the test and specifications and then being rushed or omitted due to time or contract pressures.
