Send them a picture of your eyebolts with the nice radiused transition to show them how it should be done.

There is a YT video on building rockets (I think it is by Smarter Every Day) where they talk about factors of safety. I think they said on the rockets, it is less than 1.1. This is because the cost of putting weight into space is more than the cost of doing very thorough calculations. The term is wrongly-named. It should be factor of uncertainty. The less you are absolutely sure of something, the bigger the factor has to be.

I saw something similar a few years ago. I was working on a vessel that shall be nameless, in a yard on the Seine in France that shall also be nameless.

Huge crane on the quayside, built by NOV (National Oilwell Varco). It was brand new. It was lifting some innocuous treadplate, absolutely nothing to the size of the machine. Driver boomed up and the jib contacted the ultimate stop, which didn't.

The luffing winch kept going, which sheared the jib about one third of the way up from where it was attached to the superstructure. The other two thirds of the jib went up and over the top of the crane and laid waste to the contents of the yard. The bottom third fell and destroyed a large compressor sitting below it.

Lots of debris came flying out, including whole intact sheave wheels that were a metre across. These destroyed bits on the ship including our FRC. Remarkably, no one was injured. I will never forget the bang it made.

Good crane crash..and lucky nobody was seriously hurt!
We snapped the hook off a tug in Portland Harbour, Dorset, in the 80's, it parted in just the same spot..and the Navy got excited because we were towing a bunker barge. We recovered the hook from the harbour and the artificers welded it back on FOC, nice one.

There is an element of uncertainty surrounding cranes/lifts no matter how well designed or how many H&S boxes are ticked, its only a test lift to see if its ok or it would be pressed into service as is which says it all really. Its like the crane on my truck, its more than capable of pulling the truck over, lifts are dangerous and always will be, way to many variables for them to be 100% safe.

Yes, I realised it was probably trying to lift the test piece (although you can't see any slings, chains, swivels or other means of attachment that I can make out) but why the video only started to be shot after the damage had already occurred was the puzzler. If it was so video-worthy, why not shoot the whole thing, was my point.

My guess is the full video exists but the crucial moment of failure has been edited out until "the powers that be" decide whose fault it's going to be.

went to the pub on night shift for a liquid lunch , then fell asleep in the crane i believe !!
and left the load under tension , meanwhile the tide went out and subjected the crane on the jetty to the fuĺl weight of the barge and module 2 young platers were killed as a result

We had a bridge fall on to a road with traffic on it, killing I think 6 people. Rebar joints were suspect, tied together in place with police tape, not to be removed. The Contractor came on site cut the joints loose and took them to a steel mill and tossed Them in the ladle. Just a personal opinion but drawn and quartered or hung would be called for.

Second I saw it I thought who believed this was a good idea? Unless the pontoon was sunk to the canal bed that was always going to go over, you just needed to look at it.

BAE is a joke. There’s a reason every sub is overdue and over budget.
You’ve got very very clever people, surrounded by muppets. Some of the best welders around, tied down by ridiculous rules and regulations.
I’m sure I’ve put this on here before but some time ago, they did a study at BAE.
The guys would get to work, sign out their welding consumables, by the time they got setup and welding, they done about 20m - half hour and it was tea time. Then they had to sign out fresh consumables again, then it was lunchtime, etc etc.
Everyday for two men, 3 hours of welding got done!

The reason every sub is overdue is generally down to the mod moving the goal posts every 10 minutes. I did the same job 4 different ways over a month as every time they had a meeting they decided they wanted it redone differently. Can’t say as I’ve seen welders having that issue, never ran into any drawing wire at the welding store after dinner when I was buying some fags and a bottle of pop...........

I haven’t read the whole thread @Pete. but this isn’t the worlds largest floating crane. The Saipem 7000 has a tandem lift capacity of 14000 tonnes and build many oil rigs during the late 90’s and I think it’s now the third largest in the world.

I was working on the blast furnace re-line In the early 90’s in Port Talbot steelworks when GWS cranes had the second largest crane in the world to lift the bell on to the top of the furnace after completion. The following weekend (it was a bank hol iirc) and they built up the crane to do some test lifts before moving on to the next contract in the Far East somewhere, they left it constructed and came back in the next day to find a mangled mess on the floor. They had had huge winds overnight.

When the M62 Ouse Bridge was built a crane was brought over from Holland to lift the road sections in. I believe it was the largest floating crane in the world then [1974] my grandad used to take me down to see it been built. I was only 9.

Seem to remember one of the sections falling into the river

might have been. it was in the eighteenth century (I believe), people thought the likes of Starlings forming murmurions would dive into deep water and hibernate over winter. It wasnt until this bird was found in Germany? With an arrow only used by one tribe in africa, they realised they migrated.
