This week I have been making pulleys, finished the last ones today. The radius tool is not perfect and makes plenty of noise but that will be fixed when I modify the bar.

Could rough out the groove and finish with a form tool, it would be quicker and quieter.I imagine most mass-produced pulleys are made with a CNC lathe following a programmed path, but I agree with Kram that on a big manual lathe a form tool is likely.
